This could happen through public listing medication prices. The biggest advantage ifor big Farma in the US is the federation of buyers. I think every hospital has their own contracts with farma companies.
So probably some low payed hospital emloyee, that negociates prices with multinational coorporations. Every single hospital is too small to request good prices.
But when there is a better organized institution, to buy for the same prices for all US hospitals together, they have more market power and can participate in price negotiations more actively.
In most european social states, this is working like this. And prices are significantly lower.
