Mostly I post about writing stuff here but occasionally I post about medicine. Today I’m moved to try to express how my patients restore my faith in humanity. Yesterday, Christmas Eve, I was running very behind in clinic. Many of my patients were very sick and needed extra time. By the time I saw my last patient, they had been waiting quite awhile. But there was no air of frustration when I walked in. Instead I was greeted with a warm smile, calm aura, and at the end of the visit a small gift (of no monetary value - I’m obligated to state). When we wrapped up, the clinic was empty, lights half off, staff had long gone home. After, I sat in the deserted, quiet clinic, reflecting on how every patient wished me a happy holiday, no matter what their health situation was, and how utterly humbling it is that someone in the midst of their illness would take time and effort to bring a gift (which is never expected or needed). I wish somehow to beam this genuine, deep connective, caring aspect of humanity back out into the world. Arriving home very late, my kids are now old enough to understand mom is needed by many people at work, and they made the Christmas cookies themselves. But I didn’t feel guilty this year. I just felt grateful. For my family, my patients. Take care of each other out there and happy holidays to my mastodon family here ❤️

👋oncologist here, popping on to say that stage 4 prostate cancer is not a death sentence, and there are generally well-tolerated treatments that can work well, in general, for years, just as in many other chronic diseases. Every time a celebrity or public figure is diagnosed with cancer, I observe my patients to panic over what is reported in the media. I hope the media will respect President Biden’s and his family’s privacy - he doesn’t owe an explanation of his medical situation to the media unless he wants to talk about it

All of my patients have a place in my ❤️, but the young women, many of them young mothers in their 20s and 30s, who died of cervical cancer, now preventable with the HPV vaccine, are etched in my soul. Cervical cancer commonly causes awful pain & bleeding before leading to death. We were close to eradicating this horrendous cancer. Now that’s gone poof thanks to the cowards who voted to confirm #rfkjr

I worked yesterday and today. My kids (teens) made the cookie dough and cookies on their own tonight while I sat near them and just absorbed the vibes. Then they helped wrap gifts. I’m not pretending I’m not exhausted. To moms of toddlers and younger kids, it gets easier ❤️

Here's a common "peer-to-peer" conversation I have repeatedly with the #insurance companies as an oncologist:
Me: I want to appeal your denial of my patient's PET scan
Them: Sorry, PET scan isn't on our approved list
Me: You approved it last time, and it's a standard-of-care that the patient have a repeat PET scan to assess if their treatment is working
Them: Oh, we approved it last time?
[much hemming and hawing while they verify this]
Me: By the way, are you a board-certified oncologist?
Them: [again much hemming and hawing, and only when I insist do they tell me their name and credentials (and about 95% of the time they aren't an oncologist)]
Me: Let me make sure I have the spelling of your name correct for the chart so I can make a note of who is denying the necessary medical care for my patient
Them: [Finds a way to approve the PET scan]

Something we’re taught as physicians is that when talking w/ a distressed patient, you can’t meet #emotion with information
You have to meet emotion w/ acknowledgement & validation. Only then can the emotion fade & the brain receive information
Just thinking about this in context of the U.S. election

Listen, #healthcare is so broken. I’ve been a rural oncologist for a decade, and the number of people I meet now only coming to care when the cancer is at advanced stage are overwhelming. The reduction in preventive care & screening since COVID-19 has cost people’s lives.
