Well, the exact same dynamic exists with big and smaller EMPLOYERS, competing in the market for labor: bigger corps can offer better health insurance than their employees can get from smaller companies or on their state's exchanges. Fear of losing their health insurance keeps employees from leaving jobs with big corporations: the "golden handcuffs", medical style.
