this is just some random google form. It doesn't even say what account created it. Anyone could make one just like it and scoop up email addresses, names...

I couldn't find any definitive sources with the "official link". The news stories covering it didn't seem to have one. So I signed up. I assume I will get an email at some point with more instructions...
