When people die in accidents, there's an investigation and someone may be found guilty of a crime. If that is the case for avoidable deaths due to bad decisions in child healthcare, I will have no argument. But also, that's a slope leading to tragedy piled upon tragedy - which is why civilized societies build systems designed to avoid it.
