Seems like clients aren’t even checksumming the blossom media they downloaded

yup thats the issue, you are using a blossom server, but its not in your blossom server list.
so probably clients are treating it as just another cdn. doesnt verify it.
this also means when you upload it, you dont have mirrors of it on other blossom servers.
