The NostrMCPGateway now accepts an optional `paymentOptions` field in its
options. When provided, the internal NostrServerTransport is wrapped with
`withServerPayments` to enable CEP-8 payment gating, capability advertisement,
and payment interaction negotiation. This mirrors the client-side
`withClientPayments` in NostrMCPProxy. Omitting the option keeps the server
free (non-gated).

A legacy raw-hex operator secret (LS_SK) that is not a readable 64-hex value
can never be used -- every reader gates on /^[0-9a-f]{64}$/ -- yet it lingers
looking like a usable fallback key and muddies operator diagnostics. Add a
one-off migrateOperatorStorage() run before mount that drops such junk while
leaving a salvageable (trim/lower-case) value untouched.
