I suspect that in the usual traditional pattern - family farm or small community - raw milk is probably fine; you wind up being a smallish part of a large shared microbiome smeared over you, your family members, the cows, your neighbors, .... that isn't going to permit too many pathogenic excursions (and which will recolonize you with "good" stuff if needed, say if you take a course of antibiotics); it should settle into a stable state and ***mostly*** stay there

you're ignoring the usual use case, though, I think
cars engage in a lot of stop-and-go, and short trips interspersed with longer ones
there is a cost to spinning up a turbine for that 3-mile trip to the store; what do you do with the hour the person is in the store? burn fuel for that hour (which is a total waste) to keep the turbine at speed? or do you shut it down and start it up again (wasted fuel getting back up to speed, more wear on components)?
for a very large chunk of use cases, including most city usage, the turbine shouldn't be on at all; it should only be activated when you ***know*** that it is going to need to be on for a long time, such as a long trip without stops, or when the power output can be used to do something useful (such as "refill the battery")

what you want is to avoid spinning up the turbine, though, unless it's for a long-term recharge cycle (which might include main power provision); this wastes fuel and causes wear
(remember, turbines like to be steady-state)
the goal is to have battery handle the power demand until it gets down to 33% or whatever, at which point the turbine spins up for 2 hours or however long to fully recharge it, at which point it spins down again
you really don't want the turbine spinning longer than it has to, but you also don't want it spinning up more times than it has to; there's a sweet-spot here that you want to hit, and it's almost certain to require batteries to hit it

they used to make them, but the problem with a turbine is that it likes steady and continuous, and that's not the usual power requirements for an automobile
but if you had a combination battery/capacitor/turbine all-electric powertrain, with the right management intelligence built-in, you could have something that would fuelable with basically anything from propane to diesel, would be capable of standalone self-recharging, would have great range (full battery + full fuel tank), and would have all kinds of performance
the downside would be the weight, but I have to think that would be manageable, because you could (and would have to) put the battery deck on a diet to be able to fit the turbine

the problem with "private healthcare" and "insurance" is that they wind up in a terminal death-spiral: third-party payment creates a situation where providers are incentivized to raise their prices as much as they can, because the consumer of the service is not the payer for that service and there is a fairly large amount of "float" that can be absorbed systemically without obvious specific pain felt by any particular party, therefore price discrimination is not a thing that happens

the cure for this winds up looking a lot like monarchy; the Royal College of Physicians is a prestigious thing, and titles are handed out for those who embody particular excellence, but it is ***royal*** and therefore mediated by some sovereign who takes an interest in its application to the welfare of his people

the entire "payment problem" boils down to the ugly problem of how much it is reasonable to expect somebody to pay for "reasonable" (not necessarily even "heroic") medical care over the course of a lifetime and how you can force them to do it once they consume the service; medical care is one of those goods for which there is approximately infinite demand, especially in a culture that values life, youth, and health above all else
there are all kinds of notional permutations, but they all wind up looking like something we already know about and don't want: the HMO, the insurance company, gummint healthcare, and so forth
there seems to be a limited amount of novel ideas in this equation, and some new ideas are sorely needed

yeah, the key thing to note is that (((they))) are going to be very hostile to anything which has the net effect of routing around their control and profit mechanisms, and they are very very good at sniffing those out
therefore, anything which attempts to do that will need to be very cleverly designed and implemented so that it is either illegible to their power-structures or immune to them
