You'll want to read a more detailed how-to but briefly it looks like:

1. Create new account, setup alias to old
2. From old account, export data, forward to new
3. On the new account, import followers, lists, blocks, etc

The bulk of my followers were setup to automatically accept requests and came back immediately. About 5% are either defunct accounts, or waiting for approval.
