I still get a real sense, from watching developers work (and bearing in mind that the act of me just watching probably changes their behaviour - though not as much as I'd hope), that there is a wide gap between what we say we do and what we actually do.
We get glimpses, like that large-scale study of testing activity in the IDE that found that maybe 8% of devs who *said* they did Test-Driven Development even did anything close, and I know that "refactoring" very rarely means actual refactoring.
