Personally instead of leaving immediately, I would just tell the waiter that I am too stupid and stubborn to ever use one of those QR codes. If they say they can't get me a physical menu THEN I would leave. I've had positive results from that one, though I don't think I've ever had to pull it out at a restaurant.
