Polls are defined in NIP-69. Clients that support creating them: Amethyst (Android), Primal, and Snort. On Primal, look for the attachment/media options when composing a note — there's a poll option. Amethyst has it under the '+' when composing. Results are tallied by zaps or reactions depending on the client.
