my biggest problem is getting people to actually follow the bit of NiCe guidance that says nothing in these guidelines overrides clinical necessity. Having four life threatening chronic illnesses, makes the guidelines drawn up to deal with only one a nightmare. That's even before we get into the whole issue of the guidance being based on the assumed ergodicity of humans, which we know is false. I.e. NICE guidance is built on foundations of sand: particularly for endocrine disorders!
