Regarding your questions, the key drawbacks of ever-increasing spam on the timechain (note that already about half of every block is filled with non-financial data) are that it makes it more cumbersome to run a node due to storage requirements. It may not look like a big deal today, but the trend if left unchecked could seriously limit the number of people around the world easily able to run a node, which would decrease Bitcoin’s decentralization. Another drawback is that, since Core v30, full jpegs can be added to transactions, which often include morally and legally reprehensible content. And if CSAM does make it on chain and BIP-110 fails, it could quite literally become illegal to run a node as you would have illegal material on your hard drive connected to your node. That would likely result in a trend towards centralization of Bitcoin. So yes, it matters. Run BIP-110 if you care about Bitcoin being decentralized, Bitcoin being a monetary not arbitrary file storage network, and Bitcoin being free of the most sinful, illegal material imaginable.
