One of the companies I did some work for was developing ways of preventing sexism and racism in interviews.
Think they had the best results in orchestras where they could totally hide the applicant from whoever listening so purely on how well they played.
They developed ways to anonymise cvs, this worked up till the actual face to face interviews where, of course, all the old prejudices came back.
Don't know if they made any progress with the last problem, maybe training?
