I question how much of the sluggishness in the system is due to difficulties in discharge. "Bed blocking" was a complaint routinely trotted out by Health units when I was a medical student back in the 1970s, it seems to occur with much the same frequency now.

What has significantly changed is the expected bed occupancy rate. As a student the bed manager got anxious if occupancy rose about 80%! Now I see my local DGH is reporting an occupancy of 102%....
