Perhaps we could go back to the Old Health Authority model, with a seperate Authority for primary care and fund LAs enough for Community care including Social, school, a district services. Make LAs conterminous with HAs, and re-establish the equivalent of MPC to ensure fair distribution of GPs, and RAWP to sort out the distribution of Secondary and Tertiary care. And while we're about it ensure All health professions schools are attached to Secondary rather than Tertiary centres.
