That wasn't an oops, at least not on the part of the crew. The left engine was found on the ground of the airfield where it tore off during takeoff. NTSB is investigating. But there appear to be parallels to the 1979 crash of American 191 in Chicago, where an engine broke away from a DC-10 due to a missing bolt on the pylon. Checklists and inspections seemed to prevent that from happening again, unless it happened again with this one.
