Hard to tell from black and white photos, but the cool thing about opossums is they carry their fat in their eyes - when they lose every other spot to carry excess fat, they will start to carry fat behind the eyes which cause an appearance like pic related.
So if s/he's got bulging eyes or crossed-eyes, might be fat.
