Back when Comcast headend techs built channel multiplexes in the headend someone in Colorado Springs mapped the Playboy PPV channel onto the wrong mux. It had the same program ID as Nickelodeon (of course). It went out on the system for several hours until someone in the call center actually believed a customer and called down.

After that the company policy was to map all the smut onto a prebuilt mux and dedicated equipment, with red coax and labeling. Only a few people had passwords
